Revell, Phyllis Ann (Beavers), 48, passed away July 2, 2011 at Jewish Hospital.
Funeral service will be 1pm Wednesday at Owen Funeral Home, 5317 Dixie Highway with cremation to follow.
Visitation will be Wednesday 10 until the service at 1.
