Embry, Teresa Ann, 46, of Louisville passed away peacefully at her residence on June 19, 2010.
Survivors include her daughter, Candice Bartlett; parents, Damon and Barbara Hayes Embry; brother, Neil Embry; grandchildren, Cameron and Cayden Kron; nephew, Andy Embry; and companion, William Blom
Funeral services will be private. Memorial contributions can be made to the family to assist with funeral services.
Owen Funeral Home, Dixie Highway, is in charge of arrangements.
