Adam, Jack 78 of Louisville passed away Monday, March 23, 2009 at Parkway Rehab and Nursing Center.
He is survived by his many friends and caregivers at Parkway Rehab and Nursing Center.
His Funeral Service will be 10 am Wednesday, March 25, 2009 at Owen Funeral Home-Jeffersontown
9318 Taylorsville Rd. Burial will be in Evergreen Cemetery.
Visitation will be Tuesday, March 24, 2009 from 1-6 pm at the Funeral Home.
Expressions of sympathy may be made to the Kentucky Special Olympics.
