FORMAN, Doris Jean, 89, of Louisville, passed away Wednesday, August 5, 2015.
She was a member of the old St. Clements Catholic Church, where she was a part of the Altar Society.
Jean was preceded in death by her husband, James Joseph Forman, Sr.; and daughter, Patty Alvey.
She is survived by her sons, James Jr., Rick, and Tony Forman; daughters, Jeanne Adams and Joyce Hargrove; 12 grandchildren; and numerous great and great great grandchildren.
Funeral Mass will be 10am Saturday at St. Paul Catholic Church, 6901 Dixie Highway, with burial to follow in Louisville Memorial Gardens, West. Visitation will be Friday 1pm – 8pm at Owen Funeral Home, 5317 Dixie Highway.
