Lorraine (Forman) Walczak, age 76 of Saugus, formerly of Lynn, died at the Kaplan Family Hospice House, Danvers, after a brief illness surrounded by her family. Born in Lynn she was the daughter of the late John A. and Sarah (Carroll) Forman. Before her retirement she was a waitress at several restaurants and retired from the Continental. She was the beloved mother of Kathleen M. (Walzak) Campbell of Saugus, Robert E. of Londonderry, NH, Michael E. of Saugus, Richard E. Walczak of Pompano, FL, Beloved grandmother of Shauna and Norman Campbell and Bobby and Amana Walczak. Devoted sister of Joan Forman and her husband Bill O'Donnell of Lynn, also her close cousins Sandy and her husband Chris of Rowley.
Lorraine's final wish is to pay public tribute to my two caretakers, my son Michael and sister Joan. You were both truly a "Gift from God." You went so far beyond the call of duty and your caring made feel so loved ... You are my heroes, my champions and my heart.
